
GIVE A MEMORY GIFT
Are you a healthcare professional who cares for hospice patients/ loved ones who desire to create a memory that would make their final days more meaningful?
The Lorraine Bethea Foundation may be able to assist you in fulfilling that desire through our Hospice Memory Gift Program!
To apply for this special gift the beneficiary will need to meet the following criteria:

Be an adult 18 years or older.

Be actively enrolled in hospice, with evidence of a life-limiting illness and have a life expectancy of six months or less.

Beneficiary or authorized agent should be able to articulate/communicate the beneficiary’s desire to receive the Hospice Memory Gift.

The Hospice Memory Gift has potential to positively impact the beneficiary’s quality of life
-
Beneficiary must provide evidence that he/she is physically capable of participating in the event.

Provide evidence of need for financial assistance to carry out an Event.
-
If the gift requires travel, the beneficiary/authorized agent must agree to self-fund personal effects and have a credit card available to pay for incidentals.

Beneficiary has never received a Memory Gift from the Lorraine Bethea Hospice Foundation.
